Types of Acne Scars
When it comes to acne scars, there are two types: temporary hyperpigmentation and permanent texture changes. Knowing the difference is crucial in choosing the right treatment to restore the health and appearance of your skin.
Temporary Hyperpigmentation
If you have red or brown marks after your acne has healed, this type of acne scar is known as temporary hyperpigmentation. While these marks are not permanent, they can take up to several months—and in some cases, years—to fully fade.
At Alinea Medical Spa, we use the advanced Ellipse SWT® IPL system with narrowband technology to effectively target pigmented scars and red acne marks, providing faster and more effective results than traditional laser treatments.
Aggressive Scars
Aggressive scars, or deep acne scars, are deeper marks that result from significant skin damage during the acne healing process. This typically requires more targeted treatments to repair damaged skin and create a smoother, more even complexion.
Our specialized acne scar treatment clinic in NYC treats these scars with advanced solutions like Juvéderm® and Restylane® fillers to restore smoothness and Nordlys Ellipse Fraxel Laser to remove even the most stubborn ice pick and boxcar scars.

Different Acne Scar Treatment Procedures We Offer
Laser-Based Treatments
Laser-based treatments work best for patients with severe or stubborn acne scars. At Alinea Medical Spa, our advanced laser treatments include:
- Fraxel Non-Ablative Laser Therapy: Enhances skin texture by stimulating collagen.
- Intense Pulsed Light (IPL) Therapy: Treats discoloration and redness effectively.
- Laser Skin Resurfacing: Laser Resurfacing works by treating a fraction of the skin at intervals and leaves the adjacent skin tissue intact for quick healing. The skin then becomes brighter and smoother as a result.
Exfoliation and Resurfacing Treatments
Exfoliation and resurfacing treatments are ideal for clients dealing with dull skin, acne scars, hyperpigmentation, or early signs of aging.
These treatments gently remove dead skin cells, stimulate collagen production, and enhance overall skin health. We offer the following resurfacing and exfoliation treatments:
- Chemical Peels: Chemical peels use a chemical solution that reaches deep layers to remove old and damaged skin and reveal healthier and youthful skin.
- Microdermabrasion: Removes dead skin cells for a smoother appearance.
- VI Peel: A specialized chemical peel to enhance tone and texture.
- Dermaplaning: Manual exfoliation for smoother skin.
Collagen-Stimulating Procedures
If you’re looking to improve the appearance of acne scars, tighten skin, or enhance texture without surgery, collagen-stimulating procedures are an excellent option.
These treatments help rebuild collagen, softening scar tissue and smoothing your skin for a more even complexion. Our collagen-stimulating procedures include:
- Microneedling: Microneedling is the skin-pricking process, during which your skin produces more collagen in an effort to heal small wounds from needles.
- Platelet-Rich Plasma (PRP) Therapy: Uses the body’s natural growth factors to enhance healing.
Volume Restoration Treatments
Volume restoration treatments are ideal for individuals with deep acne scars, uneven skin, or loss of facial fullness caused by acne.
By filling in depressed scars and restoring lost volume, these treatments help create a more even skin surface. They also enhance facial contours, improve texture, and boost confidence.
- Dermal Fillers: Fills deep scars and smoothens texture (e.g., Juvéderm®, Restylane®).
- Bellafill®: A long-lasting dermal filler specifically designed for acne scars, adding volume to depressed scars for a smoother, more even appearance.
Targeted Surgical Techniques
Targeted surgical techniques focus on addressing severe acne scars like boxcar, rolling, and ice-pick scars. These treatments effectively improve skin tone and create a smoother, more refined texture.
- Punch Excision and Punch Grafting: Removes and repairs deep icepick scars.
- Subcision Scar Removal: A minor outpatient procedure for depressed scars, using a special needle to release fibrous strands and stimulate collagen for smoother skin.

Common Causes of Acne Scars
Inflammatory Acne
Severe acne types like papules, pustules, or cysts trigger an inflammatory response that can damage the skin.
Clogged Pores
Pores filled with excess oil, bacteria, and dead skin cells can rupture and spill into surrounding tissue.
Skin Rupture
When the infected contents of a pore break through the skin’s surface, it damages the surrounding tissue.
Collagen Imbalance
The body’s natural healing process can overproduce or underproduce collagen, leading to uneven or discolored scar tissue.
Deep Follicular Damage
Ruptures deep in the follicular wall can cause more serious scars, such as icepick scars.
Are You a Good Candidate for Acne Scar Treatment?
- You have noticeable acne scars or marks but no active breakouts.
- You are physically healthy and suitable for minimally invasive cosmetic treatments.
- You are an older teen or adult with a stable skin condition and scarring.
- Your skin has not been recently tanned or exposed to excessive sun damage.
- You can set aside time for the recovery period required by certain treatments, such as chemical peels or laser therapy.
Acne Grades Explained:
How to Treat Each Stage Effectively
While there are two main types of acne scars, aggressive scars come in several grades, from shallow texture changes to severe indentations or raised scars that require advanced acne scar treatments. Different grades of acne scars require specialized care and treatments. Here’s a helpful guide to understanding each grade and how we approach their treatment effectively.
Grade 1 Acne Scars
Grade 1 acne scars are primarily surface-level marks that do not significantly change the skin’s texture. They are often the result of mild to moderate acne and may appear as slightly thickened areas of the epidermis.
Treatment options include IPL, HydraFacial, microneedling, and microdermabrasion, combined with chemical peels and skin-lightening ingredients like retinol, salicylic acid, kojic acid, and tretinoin.
Grade 2 Acne Scars
Grade 2 scars typically have small, shallow indentations that are visible but not deep enough to cause dramatic shadowing. They are often the result of moderate acne. Due to their gentle, wave-like appearance, these depressions are often referred to as rolling scars.
Mild resurfacing and volume restoration are ideal for treating these scars. Effective treatments include IPL, Fraxel lasers, chemical peels, microneedling, and microdermabrasion.
Grade 3 Acne Scars
Grade 3 scars are characterized by moderate atrophic (depressed) or hypertrophic (thickened) scars. Typically, this level of scarring calls for advanced combination treatments. Fraxel lasers resurface uneven skin, IPL corrects pigmentation, and fillers like Restylane® or Juvederm® restore skin volume. Our acne scar removal clinic in NYC provides all these treatments.
For icepick scars, techniques like punch excision or grafting effectively remove the damaged area, resulting in a less visible scar that blends with the skin.
Grade 4 Acne Scars
The most severe type of scarring, Grade 4 acne scars, includes deep atrophic (sunken) and hypertrophic (raised) scars that do not flatten when stretched. Common examples are boxcar scars, icepick scars, and deep pits.
These require intensive treatments such as Fraxel lasers, IPL, and chemical peels to refine the skin’s surface. Dermal fillers like Restylane® and Juvéderm® are often used to address the deepest scars and restore a more even appearance.

Acne Scar Removal FAQ
How much is laser acne scar removal in NYC?
On average laser acne scar removal in NYC costs $850 per session. Patients usually need three or more treatments depending on the severity of the acne scars. We also offer a world-class chemical peel called the ViPeel which costs $450 per treatment. Purchasing a package of treatments comes with a 10 percent discount off the entire package.
How long does it take for acne scars to fade?
It depends on the type of acne scar. Most acne scars are the result of post-inflammatory hyperpigmentation (PIH). This type of pigmented lesion can take up to two years to fade without treatment; however, eventually they will become less noticeable. True acne scars in contrast have considerable tissue damage and will need medical intervention to repair the skin.
How to get rid of butt acne scars and scars on the chest?
Treatment for acne scars on the body is similar to treating facial acne scars. This may involve Fraxel Laser, IPL photo-rejuvenation treatments, microneedling, chemical peels and medical-grade skincare products.
Does microneedling for acne scars work?
Microneedling is excellent for treating Grade 1 and Grade 2 type scarring (mild to moderate). A series of four to eight microneedling sessions may be required for noticeable improvement in skin smoothness and acne scar amelioration. For deeper or more severe scars, Fraxel non-ablative laser resurfacing, IPL and chemical peels be ideal.
Can I use a dermaroller for acne scars?
A dermaroller is a manual microneedling device sold for home use. Although these devices may have some benefit, in amateur hands they are more likely to cause complications such as infections and skin tearing. For serious acne scar treatments, we advise you to seek guidance from a qualified skincare professional.
How many microdermabrasion treatments do I need for acne scars?
In most cases microdermabrasion alone is not sufficient to resolve acne scars. You may first need to undergo deeper resurfacing techniques such as Fraxel laser or IPL photo-rejuvenation to resolve texture and pigmentation issues. A series of four to six microdermabrasion sessions can be used as a maintenance program after your laser treatments to keep skin smooth and healthy.
